Do we accept health insurance?

No, we do not. However, if you choose to do so, you may submit your charge sheet to your insurance company for potential reimbursement (paid directly to you), or for application toward your deductible (“out of pocket” expenses), since we are considered “out of network” providers.*

No shows, late cancellations, and last minute rescheduling

Please be aware that we have fees for no-shows, cancellations, and rescheduled appointments made within 24 hours of your appointment time. For initial appointments, the fee is $80 to cover the provider’s and healthcare team’s time. For existing patients, the fee is $50. We understand this can be frustrating, but it’s important to respect the time of our medical team. However, we acknowledge that emergencies happen. If you experience an emergency, please contact us at hello@avivahealthwellness.com or send us a message through your patient portal, and our scheduling team will assist you.

Where and how do patients get their labs?

We have several options available to you. We are always trying to make things as convenient as possible.

Schedule your labs to be drawn at Aviva. We use Bioreach lab systems (cash pay only). Lab results typically back in 1-2 days. Bioreach has the option to schedule a phlebotomist to come to you. Whether at work or at home. (extra fee of $75 for this service).

You would schedule this service through Aviva by calling the office. (cash pay only) Lab results typically come back in 1-2 days.

Take your lab order (that we provide) to an outside lab. If you use an outside lab source YOU are responsible for emailing/faxing/bringing your labs to your Aviva providers. DO NOT assume that they will send them to us…as this isn’t typically the case.

Outside lab sources MAY accept your insurance or offer cash pay options as well. You will need to contact the individual lab for this information.

Outside lab sources can take up to 2 weeks for labs to result. Please plan your lab review accordingly so we can go over your results AT the appointment. If the labs have not resulted you will be asked to reschedule. Frequently used options:
